This looks somewhat like what Dayrat (2010) calls "Montereina" sp. F. The initial specimen in the first two shots below was found in an eastern lagoon reef Halimeda patch at a depth of 10m.
A second specimen, measuring 33mm, was found in the same general area on 7 September 2008.
